# (2.3.9) Creating Task for Employee on Permanent (Disciplinary Action)

### Precondition <a href="#precondition" id="precondition"></a>

1. Choose the ***Permanent*** module from the main menu on the left side.
2. Click ***All***  key.
3. Choose the employee you want to ***Disciplinary Action***.
4. Click the ***Disciplinary Action*** task.

You will see the Disciplinary Action form as following:

* Choose the responsible user (the ***Assignee***) from the Assign To drop down list.&#x20;
* Fill in these fields: ***Effective Date, Start Date, Amount, Punishment Type, Punishment Level, Reason.***
* User can select ***Punishment Type*** (First Warning, Second Warning, Third Warning) at drop down list.
